Síndrome Compartimental Aguda das Extremidades

Resumo

A síndrome compartimental acontece quando a homeostase de um compartimento é comprometida, seja pelo aumento da pressão interna ou pela incapacidade de distensão do mesmo. Na impossibilidade de aliviar a pressão do compartimento, há lesão de suas estruturas com consequências devastadoras se o quadro não for diagnosticado e tratado a tempo. É preciso que o médico esteja alerta à possibilidade de uma síndrome compartimental e para tal deve estar familiarizado com a história clínica e a evolução da condição. O exame físico costuma ser suficiente para chegar ao diagnóstico, mas exames complementares podem ser necessários, sejam eles invasivos ou não. A elevação do membro, retirada de vestimentas compressivas e descompressão cirúrgica (fasciotomia) fazem parte do tratamento adequado.

Abstract

Compartment syndrome occurs when the homeostasis of a compartment is compromised, either by increased pressure or by the inability to distend it. If it is impossible to relieve the pressure in the compartment, there is damage to it’s internal structures with devastating consequences if the condition is not diagnosed and treated in time. It is necessary for the physician to be alert to the possibility of the compartment syndrome and, for that, he must be familiar with the clinical history and evolution of the condition. Physical examination is usually sufficient to reach the diagnosis, but additional tests may be necessary, whether invasive or not. Limb elevation, removal of compression garments and surgical decompression (fasciotomy) are part of the appropriate treatment.
