A Importância da Análise do Linfonodo Sentinela no Prognóstico do Melanoma Cutâneo

Resumo

Introdução: O melanoma cutâneo é uma neoplasia maligna originada dos melanócitos e tem origem neuroectodérmica. Embora apresente baixa incidência, possui alta letalidade pelo seu alto potencial de metástases. A disseminação das metástases ocorre principalmente por via linfática, sendo definido como linfonodo sentinela o primeiro linfonodo a drenar o tumor primário, o qual geralmente é local de implantação do tumor. A positividade do linfonodo sentinela é o principal determinante do prognóstico, caracterizando evoluções mais graves. Objetivos: Avaliar a importância da análise do linfonodo sentinela no prognóstico dos pacientes com melanoma cutâneo. Metodologia: Revisão de Literatura sobre o tema a partir de artigos científicos, nas plataformas Scielo, Google Acadêmico e PubMed, através das palavras-chaves: “melanoma cutâneo”, “melanoma”, “melanoma tratamento” e “linfonodo sentinela”. Discussão: A técnica de identificação do linfonodo sentinela viabiliza a análise histológica, capaz de identificar com alto grau de precisão, pacientes com melanoma inicial com metástases nodais. A partir de diversos estudos baseados no índice de Breslow, que relacionam a espessura da lesão com a positividade do linfonodo sentinela, a biópsia deste demonstra efetividade para prever recorrências de melanoma cutâneo, como também para avaliar a sobrevida do paciente. Conclusão: A positividade do linfonodo sentinela associada a biópsia do mesmo, permite o estadiamento da doença. O estudo ressalta a importância de um diagnóstico de alta precisão e precoce, além de manejo adequado da lesão inicial por meio da análise do linfonodo sentinela. 

Abstract

Introduction: The cutaneous melanoma is a malignant neoplasm originated from melanocytes and has neuroectodermal origin. Although it presents low incidence, it shows high lethality because of your high metastatic potential. The metastatic dissemination mainly occurs by lymphatic way, thereby the first lymph node to dry the primary tumor is the sentinel lymph node, the one of which is usually the spot of tumor implantation. The positivity of the sentinel lymph node is the main determinant of the prognostic, featuring more dangerous evolutions. Objectives: To evaluate the importance of the analysis of the sentinel lymph node to the prognostic of cutaneous melanoma. Methodology: This is a literature review about the theme starting of scientific articles, on the platforms Scielo, Academic Google e PubMed through the keywords “melanoma”,“cutaneous melanoma”, “treatment melanoma” and “sentinel lymph node”. Discussion: It was developed as a technique of identification of sentinel lymph nodes, that increases the histologic analysis, which can identify high degrees of precision patients with initial melanoma and metastatic nodes. Based on several studies through the Breslow’s indices, that connect the lesion thickness with the positivity of the sentinel lymph node, the biopsy of the sentinel lymph node shows itself proper to predict the recurrence of cutaneous melanoma as well to evaluate the survival of the patient. Conclusion: The positivity of the sentinel lymph node associated with the biopsy of the sentinel lymph node applies to the staging of the disease. The study highlights the importance of a precocious and accurate diagnosis, besides an appropriate management of the initial lesion through the analysis of the sentinel lymph node.
